It acts by relaxation of vascular smooth muscle; consequently it dilates peripheral arteries and veins. It is more active on veins than on arteries. It reduces both preload & afterload which will reduce workload on the heart. In hypertensive patients, moderate doses induce renal vasodilation roughly proportional to the decrease in systemic blood pressure; thus there is no appreciable change in RBF and GFR. Hypotensive effect is seen within a minute or two after the start of an adequate infusion, and it dissipates almost as rapidly after infusion is discontinued.
Hypertensive crisis, Congenstive heart failure, Controlled hypotension, acute mitral regurgination.
It is given parenterally and onset of action occurs quickly (within 1 minute). On stopping I.V. infusion, the effect dissipates rapidly.
Compensatory hypertension.
None except in overdosage. Nausea, retching, apprehension, headache, restlessness, muscle twitching, dizziness.
Sodium nitroprusside decomposes on exposure to light, so the container must be protected from light.
Adults
: Heart failure : by i.v. infusion initially 10-15mcg / kg/ minute, increase every 5-10 minutes as necessary, usual range 10-200mcg / kg / minute.
3 - 5 minutes.
50mg / ml, 5ml vial.
