 Action :

Doxorubicin is a cytotoxic anthracycline antibiotic. The cytotoxic action results from its binding to DNA and inhibition of nucleic acid synthesis. Doxorubicin has been shown to produce regression in a variety of disseminated malignancies.

  

 Indication :

GI tract carcinoma. Acute myeloblastic leukaemia, bronchogenic, breast and ovarian carcinoma, soft tissue and bone sarcomas, malignant lymphoma. Primary management of nonmetastatic bladder carcinoma (intravesical administration). Wilm’s tumor. Neuroblastoma.

  

 Pharmaco-Kinetics :

It is usually administered by intravenous route. It does not cross blood brain barrier.

  

 Contraindication :

Buccal ulceration, bone marrow depression. Preexisting heart disease.

  

 Adverse-Effects :

Nausea, vomiting, diarrhoea, fever, red urine (harmless), ventricular arrhythmia, severe local tissue damage on extravasation, cardiotoxicity, bone marrow depression, anorexia, stomatitis, alopecia, conjunctivitis.

  

 Special Precaution :

Cardiac or hepatic dysfunction. Haematological and cardiac monitoring, children, hyperuricaemia, infection.

  

 Interactions :

Mercaptopurine

:

Hepatotoxicity of mercaptopurine enhanced leading to cholestasis.

Cyclophosphamide

:

Exacerbation of cyclophosphamide induced hemorrhagic cystitis.

Barbiturates

:

Increase the plasma clearance of doxorubicin.

Digoxin

:

Decreased serum levels of digoxin.

Streptozotocin

:

Increase toxicity of doxorubicin.

Radiation

:

Radiation induced toxicity to myocardium, mucosa, skin and liver increased.

  

 Dosage :

Adults :  I.V. :  60 - 75mg / m2 single dose every 21 days or 30mg / m2 daily every 3 days. Repeat after 4 weeks. Maximum dose :  550mg / m2.

     

 Duration of action :

Elimination half-life 12-23 hours.

    

 Strength & Packing :

10mg /   5ml  Injection, 
50mg / 25ml  Injection.
