Aminophylline is a soluble complex containing 79% of anhydrous theophylline and 13.5-15% of ethylenediamine. The letter is inactive and does not contribute to bronchial relaxation. Theopylline directly relaxes bronchial smooth muscle as also pulmonary blood vessels. Suggested mechanisms for this action are an increase in intracelluar cyclic AMP through inhibition of phosphodiesterase, prostaglandin antagonism, an effect on translocation intracellular Ca2+, stimulation of endogenous catecholamines and inhibition of cGMP.
Reversible airways obstruction, severe acute asthma.
The following solutions should not be mixed with aminophylline in i.v. fluid. Chlorpromazine, dobutamine, epinephrine, norepinephrine bitartrate, insulin, oxytetracyckine, penjcillin G potassium, phenobarbitol, phenytoin, prochloperazine, promazine, vancomycin, verapamil, vitamin C, vitamin B complex with C.
