Vinblastine  Sulphate   -   Antineoplastic

  

 Action :

Interferes with metabolic pathways of amino acids leading from glutamic acid to the citric acid cycle and urea. Vinblastine has an effect on cell energy production required for mitosis and interferes with nucleic acid synthesis. Reversal of anti tumour effect by glutamic acid or tryptophan has occurred.

  

 Indication :

Hodgkin's & non Hodgkin's lymphomas, mycosis fungicides, testicular carcinoma, kaposi's sarcoma.

  

 Pharmaco-Kinetics :

It undergoes rapid distribution and extensive tissue binding following I.V. injection. It is metabolised in liver.

  

 Contraindication :

Leucopenia, presence of bacterial infection, significant granulocytopenia unless it is the effect of the disease itself.

  

 Adverse-Effects :

Hepatic function impairment, leucopenia, aspermia. Nausea, vomiting, hypertension. Alopecia.

  

 Special Precaution :

The needle should be properly position in the vein as any leakage may cause considerable irritation. Fatal if given intrathecally.

  

 Interactions :

Mitomycin

:

Acute shortness of breath and severe bronchospasm.

Phenytoin

:

Combination chemotherapy (including vinblastine) may reduce plasma levels and increase seizure activity.

Erythromycin

:

Severe myalgia, neutropenia and constipation.

  

 Dosage :

Adults

:

For I.V. use only.

Initial : 3.7mg / m2 single dose. Increments at weekly intervals depending on WBC counts (counts should not come below 3000 / mm3).

Maximum dose :  18.5mg / m2.

Children

:

Initial 2.5mg/m2 single dose. Increments at weekly intervals depending on WBC counts.

Maximum dose :  12.5mg / m2.

       

 Strength & Packing :

1mg Injection, single vial per box.
