 Action :

It is bactericidal drug and it exerts its action by inhibiting the synthesis of the cell wall in sensitive bacteria.

  

 Indication :

In seriuos life threatening staphylococcal infections resistant to other antibiotics. In severe staphylococcal infections in patients who are allergic to Penicillins and Cephalosporins. In Endocarditis caused by Enterococcus faecalis vancomycin is used in combination with aminoglycoside. Prophylaxis against bacterial endocarditis in Penicillin allergic patients with CHD, rheumatic heart disease or other valvular heart disease when they undergo surgical procedure of upper R-tract or dental surgery.

  

 Pharmaco-Kinetics :

After oral administration it is poorly adsorbed. It is given by parentral route and high concentration of drug may accumulate when renal function is imparied.

  

 Contraindication :

Hypersensitivity to Vancomycin.

  

 Adverse-Effects :

Hypersensitivity reactions e.g. skin rash and anaphylaxis. Local pain and phiebitis at site of intravenous injection. Nausea, chills, fever, ototoxicity and nephrotoxicity. Hypotension, eosinophilia.

  

 Special Precaution :

Renal impairment, previous hearing loss, to monitor renal and auditory functions and blood levels. Children. Rapid or bolus administration

  

 Interactions :

Anion-exchange resins

:

Antagonism of oral vancomycin by cholestyramine.

Aminoglycosides

:

Risk of nephorotoxicity may be increased.

Anaesthetics

:

Erythema and histamine like flushing in children.

Nondepolorizing
Muscle Relaxants

:

The neuromuscular blockade may be enhanced.

  

 Dosage :

Adults

:

2gm daily in 2-4 divided doses.

Children

:

10mg/kg every 6 hrs.

Infants & neonates

:

Initially 15mg/kg followed by 10mg/kg, 12 hourly in 1st week of life and then 8 hourly upto 1 month age.

  

 Duration of action :

6-8 hours.

  

 Strength & Packing :

500mg and  1gram  vial.
