Urokinase  -  Thrombolytic

 Action :

A thrombolytic agent obtained from human kidney cells by tissue culture. Urokinase directly converts plasminogen into plasmin which then degrades fibrin clots as well as fibrinogen and other plasma proteins.

  

 Indication :

Acute myocardial infraction. Pulmonary embolism, deep vein thrombosis (DVT), peripheral vascular thrombo-embolism.

  

 Pharmaco-Kinetics :

  

  

 Contraindication :

Surgery within 10days, severe hypertension, serious gastrointestinal bleeding, hemorrhagic disorder, previous cerebrovascular accident.

  

 Adverse-Effects :

Superficial bleeding at sites of venous cutdowns, vascular punctures etc may occur. More seriously internal bleeding could occur in any internal organ or site. Relatively mild allergic reactions such as bronchospasm or rash may occur occasionally. Other side effects include chills, BP changes, dyspnoea, palpitation, cyanosis & hypoxemia. Aspirin should not be given for fever.

  

 Special Precaution :

Any vascular punctures should be avoided as far as possible and if unavoidable done with great care. Upper extremities preferable for arterial punctures because pressure application is easier. Caution in conditions such as possibility of a left heart thrombus, sub acute bacterial endocarditic, coagulation defects, cerebrovascular disease, diabetic retinopathy, pregnancy, and any condition in which bleeding could be dangerous. Monitor for reperfusion arrhythmias.

  

 Interactions :

Anticoagulants and antiplatelet agents. Carefully monitor since they may cause bleeding complications. 

  

 Dosage :

  
IV  Infusion :
2,000,000 I.U. as bolus. Alternatively 2,000,000 I.U. as IV infusion over one hour or 10,000 I.U. per minute by intracoronary route. Clot is usually used within 30 mins. of IV route or one hour of intracoronary route.

Pulmonary embolism :  1st day 50,000 I.U. in 10ml saline or 5% dextrose given I.V. over 10mins. followed by 3000,000 I.U. in 200ml saline or 5% dextrose over 8-12 hrs. Then 50,000 I.U. daily for 6-13 days.

  

 Onset of action :

Within mins. 

  

 Duration of action :

Few hours
