 Action :

Forms a complex with plasminogen which then converts plasminogen to plasmin. Plasmin breaks down clots as well as fibrinogen and other plasma proteins.

  

 Indication :

Acute myocardial infarction. Pulmonary embolism. Deep Vein thrombosis. Arterial thrombosis. Acute thrombosis of central retinal vessels, extensive coronary emboli, severe ileofemoral thrombophlebitis.

 

 Pharmaco-Kinetics :

It is given by parental route and has a short plasma half life.

    

 Contraindication :

Surgery within 10 days, severe hypertension, serious gastrointestinal bleeding, haemorrhagic disorder, previous cerebrovascular accident.

  

 Adverse-Effects :

Fever, allergic reactions, bleeding from different sites, rarely anaphylaxis, arrhythmias, bronchospasm.

  

 Special Precaution :

Sub-acute bacterial endocarditic parturition, kidney or liver impairment.

  

 Interactions :

Anticoagulants and antiplatelet agents

:

May cause bleeding complications. Therefore patients should be carefully monitored.

Lab Tests

:

Marked decrease in plasminogen and fibrinogen. Increase in thrombin time (TT), activated partial thromboplastin time (APTT), & prothrombin time (PT).

   

 Dosage :

Adults : 
Acute myocardial onfarction :  I.V.  750,000 - 15 million I.U. in 1 hour. 
Intracoronary : 20,000 I.U. stat followed by 2000 - 4000 I.U./ min. for 30 - 90 min. 

  

 Duration of action :

Hyper-fibrinolytic effect lasts for few hours. 

    

 Strength & Packing :

1,500,000 I.U.,  single vial pack.
