Nitroglycerine (Glyceryl Trinitrate) -  Nitrate Antianginal

 Action :

Glyceryl trinitrate relaxes smooth muscle especially in the blood vessels. Reduces venous return (preload) and facilitates subendocardial blood flow with redistribution into ischaemic areas. Relieves coronary vasospasm. Dilates arterioles and reduces afterload. Useful for relief and prevention of anginal attacks.

  

 Indication :

Tablets

:

Angina pectoris.

I.V.

:

 Unstable angina, coronary vasospasm, LVF accompanying MI, hypertension during cardiac surgery.
Ointment & Transdermal patch : Prevention of angina pectoris, either alone or in combination with other antianginal agents.

   

 Pharmaco-Kinetics :

Administered by sublingual route it is absorbed rapidly.

    

 Contraindication :

Cerebral trauma, cerebral haemorrhage, hypertrophic cardiomyopathy.

  

 Adverse-Effects :

Throbbing headache, weakness, sweating, flushing, dizziness, tachycardia, palpitation, methaemoglobinaemia, fainting.

  

 Special Precaution :

Pulmonary disease, cerebrovascular disease.

  

 Interactions :

Same as nitrates.

   

 Dosage :

Adults : 
Inj. : Initially 5mcg / minute through infusion. Increase with increments of 5mcg / minute.
SR/CR : 2.5 - 6.5mg  8 - 12 hourly. 

  

 Duration of action :

SL : 30 - 60 mins.;   I.V. : 3 - 5 mins.;   SR : 3 - 8 hours.
