Metoclopramide  -  Prokinetic

  

 Action :

Metoclopramide blocks central and peripheral dopamine receptors. The latter activity results in stimulation of plain muscle in the stomach and upper GI tract. This results in rapid gastric emptying and faster intestinal transit. Lower oesophageal sphincter pressure is increased and this prevents oessophageal reflux . Central dopamine inhibition abolishes nausea and vomiting. Prolactin levels are increased and extrapyramidal side effects may be seen.

  

 Indication :

Nausea, vomiting. Gastritis. Reflux oesophagitis. Hiccups, Lactation failure. Post operative vomiting. Diagnostic procedure in gastroenterology. Hiccups.

    

 Contraindication :

G. I. obstruction or perforation, epilepsy, breast cancer and extrapyramidal symptoms.

  

 Adverse-Effects :

Drowsiness, facial spasm, trismus, oculogyric crisis seen commonly in children and young adults.

Tardive dyskinesia in elderly on prolonged use, gynaecomastia, galactorrhoea, diarrhoea and restlessness.

  

 Special Precaution :

IV injection to be administered slowly to avoid restlessness and anxiety.

  

 Interactions :

Anticholinergic drugs and narcotic analgesics

:

Effects on GI mortility are antagonised by these drugs.

Alcohol, sedatives, hypnotics, narcotics or tranquilizers

:

Additive sedative effects may occur.

Digoxin, Cimetidine

:

Absorption of these drugs is decreased.

Acetaminophen, Aspirin

:

The absorption of these agents is increased.

Phenothiazines, Butyrophenone, Lithium and Thioxanthine drugs

:

May potentiate extrapyramidial effects.

Bromocriptine

:

Antagonism of hypoprolactinaemic effect of bromocriptine.

Cyclosporine

:

May lead to increased cyclosporine absorption, possibly increasing immunosuppressive and toxic effects.

Succinyl choline : Metoclopromide may increase the neuromuscular blocking effects of succinyl choline.

  

 Dosage :

Adults : 
Tabs. : 5 - 10mg thrice daily.
Inj. : 2ml  2 - 3 times daily, if required.

  

 Duration of action :

1-2 hours.
