 Action :

Inhabits dihydrofolic acid reductase and therefore interferes with DNA synthesis and cell replication. Actively multiplying cells such as malignant cells, bone marrow, foetal cells, buccal and intestinal mucosa and cells in urinary bladder are more sensitive. In patients with rheumatoid arthritis, Methotrexate reduces joint swelling and tenderness.

  

 Indication :

Lympholastic leukaemia & choriocarcinoma, Hydatidiform mole. Psoriasis, R.A. : (in adults) Adjuvant therapy of non metastatic osteosarcoma. To reduce corticosteroid requirements in patients with severe corticosteriod dependent asthma.

  

 Pharmaco-Kinetics :

After oral administration it is readily absorbed from G.I. tract but larger doses are absorbed incompletely, little drug is metabolised and it is excretd largely unchanged in urine.

  

 Contraindication :

Severe hepatic or renal impairment, severe anaemia, thrombocytopenia or leucopenia.

  

 Adverse-Effects :

Nausea, vomiting, diarrhoea, anaphylaxis, hepatic necrosis, fever, bone marrow depression, osteoporosis, menstrual dysfunction, cirrhosis, pulmonary infilterates and fibrosis, renal toxicity, depigmentation.

  

 Special Precaution :

Children, GI disorders, CNS disturbance, hepatic or renal impairment, bone marrow depression. Monitor hepatic, renal and haematological functions.

  

 Interactions :

Aminoglycosides

:

Decrease absorption and AUC of oral Methotrexate.

Charcoal

:

Decreases serum level of both oral and IV Methotrexate.

Etretinate

:

Hepatotoxicity.

Folic acid or its derivatives

:

Decrease response to methotrexate.

NSAIDs

:

Increase plasma levels of Methotrexate which may be fatal.

Phenytoin

:

Serum concentration of phenytoin decreased.

Probenecid, Salicylates & Sulfonamides

:

Increase in effects of methrotrexate.

Procarbazine

:

Increases nephrotoxicity of Methotrexate.

Thiopurines

:

Increase AUC and plasma levels of thioprurines.

Asparaginase

:

Reduces toxicity.

Food

:

Reduction in plasma concentration of Methotrexate.

  

 Dosage :

Leukemia

:

Maintenance of remission

:

30mg/m2  I.M. twice a week.

Intrathecal dose

:

12mg / m2 maximum upto 15mg.

Choriocarcinoma

:

15-30mg orally or I.M. daily 5 days repeat after 1 week. 
Repeat for 3-5 courses.

  

 Duration of action :

Terminal elimination half-life 3-15 hours.  

    

 Strength & Packing :

50mg / 2ml Injection (per vial).
2.5mg tablet, 100 tablets per bottle / blister
