Melphalan   -   Alkylating Antineoplastic /  Cytotoxic

  
Melphalan Tablets

  

 Action :

An alkylating agent active against some malignancies. It is active against both resting an rapidly dividing cells.

  

 Indication :

Breast carcinoma, multiple myeloma, advanced ovarian carcinoma, other neoplasms in combination with surgery. Malignant melanoma, polycythaemia vera.

  

 Pharmaco-Kinetics :

After oral administration it absorbed in incomplete and variable manner.

  

 Contraindication :

Patients whose disease has demonstrated prior resistance to this drug.

  

 Adverse-Effects :

Nausea, allergic reactions, thrombocytopenia, bone marrow depression, inappropriate secretion of ADH, amenorrhoea, leukaemia, sterility, pulmonary infiltrates.

  

 Special Precaution :

Haematological monitoring, leukaemogenic.

  

 Interactions :

Cyclosporine

:

Potential risk of severe renal failure.

Carmustine

:

The carmustine lung toxicity threshold may be reduced.

Cisplatin

:

Induces renal dysfunction resulting in altered melphalan clearance.

Nalidixic acid

:

May cause severe haemorrhagic necrotic enterocolitis in children.

  

 Dosage :

Adults

:

Multiple myeloma   :  usual oral dose 6mg / day.

Maintenance dose :  2mg / day. Alternatively 7 days course of 10mg daily. Another course not until recovery of blood counts, particularly neutrophils and platelets.

Children

:

Multiple myeloma   :  0.15mg / kg / day for 7 days.

Maintenance dose :  0.05mg / kg / day when WBC / platlets count are rising.

Ovarian carcinoma :  0.2mg / kg / day for 5 days. Repeat course after 4-5 weeks.

  

 Onset of action :

Few mins. to 6 hours.

 

 Duration of action :

Terminal elimination half-life 90 mins.

    

 Strength & Packing :

2mg tablet,  
5mg tablet,  25 tablets per bottle.
