Bupivacaine   -   Local Anaesthetic

  

 Action :

Blocks the generation and conduction of nerve impulses. Order of loss of function is as follows : Pain, temperature, touch, proprioception and skeletal muscle tone. Toxic levels may depress cardiac conduction, excitability and cause AV block, ventricular arrhythmias and cardiac arrest.

  

 Indication :

Infiltration anaesthesia, epidural and spinal anaesthesia, nerve block.

  

 Contraindication :

Digital block and I.V. regional anaesthesia.

  

 Adverse-Effects :

Cardiac arrest, cardiac arrhythmias, respiratory failure.

  

 Special Precaution :

Heart block, hepatic dysfunction, shock, heart disease, debilitated patients.

  

 Interactions :

Cimetidine & Ranitidine

:

Clearance of Bupivacaine decreased.

Lignocaine & Mepivacaine

:

Efficacy potentiated by bupivacaine.

  

 Dosage :

Maximum dose is 150mg in a 4 hour period.
