Bleomycin Sulphate   -   Antineoplastic  drug

  

 Action :

Main mode of antineoplastic action is inhibition of DNA synthesis and to a lesser extent that of RNA and protein synthesis. Causes very little bone marrow suppression.

  

 Indication :

Ademocarcinoma, lymphosarcoma and seminoma. Superficial bladder cancer as a adjuvant in primary or recurrent pterygia.

  

 Pharmaco-Kinetics :

It is usually administered by parenteral route. It does not cross blood brain barrier.

  

 Contraindication :

Haemorrhagic tendencies, bone marrow depression, not to be given I.M. or S.C., thrombocytopenia.

  

 Adverse-Effects :

Nausea, vomiting, allergic reactions, fever, anaphylaxis, skin rash, raynaud’s phenomenon, stomatitis, pulmonary fibrosis, hyperpigmentation. Renal and hepatic toxicity.

  

 Special Precaution :

Lymphoma patients, monitor pulmonary function, anaesthesia.

  

 Interactions :

Digoxin

:

Decreased serum levels of digoxin.

Phenytoin

:

Decrease serum levels of phenytoin.

Radiatherapy

:

Enhanced toxicity.

O2 Therapy

:

Enhanced pulmonary toxicity.

Cisplatin

:

Toxicity increased by bleomycin.

Vincristine

:

Debilitation syndrome.

  

 Dosage :

Adults

:

30mg twice weekly in average cases, otherwise dose can be varied between 15mg daily to 15mg weekly.

Total dose : 300 - 400mg.

Small cell cancer   :  0.25mg - 05mg / kg I.V.

Hodgkin’s disease :  0.25 - 0.5mg / kg I.V., I.M., S.C.

   

 Duration of action :

Terminal elimination half-life 2 - 4 hours.

    

 Strength & Packing :

15mg Injection,  single vial per box.
