Acyclovir  -  Anti-Viral

  
Manufacturers of Acyclovir Injection - Tablets - Cream, an Antiviral drug, used in treatment of herpes,

  

 Action :

Acyclovir exerts a highly sensitive inhibitory action against herpes simplex types 1 and 2, Varicella - Zoster virus, Epstein-Barr virus and Cytomegalo virus. The enzyme thymidine kinase(TK) in virus-infected but not uninfected cells initiates the activation of acyclovir into the triphosphate. This interferes with viral DNA polymerase and inhibits viral duplication.

  

 Indication :

Treatment and long term suppression of herpes simplex infections of skin and mucous membranes (genital herpes simplex, mucocutaneous H.simplex and chickenpox). Prophylaxis in immuno-compromised patients. Treatment of herpes zoster. Herpes simplex encephalitis, H. simplex keratitis.

  

 Pharmaco-Kinetics :

After oral administration acyclovir is excreted primarily unchanged in the urine. Hence in renal dysfunction dose is to be decreased.

  

 Contraindication :

Hypersensitivity to acyclovir or any component of the formulation.

  

 Adverse-Effects :

After topical application irritation, burning sensation and stinging may occur. Nausea, malaise, headache. I.V. administration may cause skin rash, sweating, hypotension and vomiting. CNS effects include hallucination, seizure and coma which are due to high dose but are reversible.

  

 Special Precaution :

Renal dysfunction.

  

 Interactions :

Probenecid

:

Increases efficacy by decreasing urinary excretion and increasing half-life.

Zidovudine

:

Causes severe drowsiness and lethargy.

  

 Dosage :

  
ORAL :

Herpes Simplex Initial therapy : 200mg  5 times daily for 10 days.
Herpes Simplex Chronic, Supressive therapy : 200 - 400mg  2 to 4 times daily for 12 months.
Re-evaluate before continuation of therapy.
Herpes Zoster : 800mg  5 times daily at 4 hours intervals for 7-10 days.
Varicella Zoster : 20mg/kg (not more than 800mg) 4 times daily for 5 days.

  

IV  Infusion :

Herpes Simplex Encephalitis : 10mg/kg infused at constant rate over 1 hour every 8 hours for 7 days in adults with normal renal function. 
Children :  6 months to 12 years 500mg/m2 infused over 1 hour every 8 hours for 7 days.
Severe Episodes of Herpes Genitalis : 5mg/kg infused over 1 hour every 8 hours for 5 days in adults.
Children :  Under 12 years 250mg/m2 infused over 1 hour every 8 hours for 5 days.

  

 Onset of action :

within 24 hours

  

 Duration of action :

upto 8 hours. 

  

 Strength & Packing :

250mg  Injection Lyophilized - Ready for use (single vial per box), 
200mg, 400mg,   800mg  tablet  (10 tablets per box).
     5gm  5% w/w  Cream.
